STEP 1 - NUTRITION ASSESSMENT

ALL NEW PATIENTS MUST COMPLETE A FULL NUTRITION ASSESSMENT SESSION

FULL NUTRITION ASSESSMENT SESSION WITH A LICENSED REGISTERED DIETITIAN

  • 1 Registered Dietitian nutrition assessment session (up to 60-minutes in length) conducted via Webcam, Telephone, or Email counselling, for Ontario patients only
  • Foundational resources + grocery tour guide, sent via-email to all new patients
  • Discussion about goal recommendations for patients before starting a nutrition plan
  • Discussion with the Registered Dietitian about continuation after the assessment

$120.00 + HST (paid in full at end of session - before exiting)

(**insurance invoice is provided once payment is submitted)

Please note: The Registered Dietitian will recommend the right Treatment Plan Bundle option for the patient moving forward, based on the assessment and the professional opinion of the Registered Dietitian (regulated service by The College of Registered Dietitians of Ontario). STEP 2 - TREATMENT PLANS

AFTER COMPLETING A FULL NUTRITION ASSESSMENT SESSION, THE REGISTERED DIETITIAN WILL RECOMMEND A TREATMENT PLAN FOR THE PATIENT

LONG-TERM TREATMENT PLAN BUNDLE (adult patients) - $300.00 + HST

  • 3 Licensed Registered Dietitian sessions (each up to 40-minutes in length) conducted via Webcam, Telephone, or Email Counselling
  • 4 Personalized Meal Plans based on the nutrition assessment session and the patient's goals (12 meal ideas/recipes, and 4-6 different snacking options) - can be used for 2-3 months by patients 
  • Supporting resource documents included (i.e., mindful eating guide, intuitive eating guide, sports nutrition guide, sleep and nutrition guide, etc.).
  • This bundle provides comprehensive service for patients who need assistance with 2-3 nutrition goals
  • This bundle is best for patients over the age of 16 (adult patients)

Long-Term Treatment Plan Bundle Cost:
$300.00 + HST (paid in full upon start of the bundle - insurance invoices are split)

Insurance Invoice Breakdown - Patients Receive Three in Total:
Session 1 = $100.00 + HST
Session 2 = $100.00 + HST
Session 3 = $100.00 + HST

(**insurance invoices are provided after each session is rendered, by law - payment due when the bundle starts)

SHORT-TERM TREATMENT PLAN BUNDLE (pediatric patients + returning patients) - $175.00 + HST

  • 2 Licensed Registered Dietitian sessions (each up to 30-minutes in length) conducted via Webcam, Telephone, or Email Counselling
  • 2 Personalized Meal Plans based on the nutrition assessment session and the patient's goals (6 meal ideas/recipes, and 2-4 different snacking options)
  • Supporting resource documents included (i.e., mindful eating guides, intuitive eating guide, sports nutrition guide, etc.).
  • This bundle is best for patients who are continuing their care at the clinic, or who have already worked closely with a Registered Dietitian in Ontario
  • This bundle is best for patients under the age of 16 (pediatric patients)

Short-Term Treatment Plan Bundle Cost: 
$175.00 + HST (paid in full upon start of the bundle - insurance invoices are split)

Insurance Invoice Breakdown - Patients Receive Three in Total:
Session 1 = $100.00 + HST
Session 2 = $75.00 + HST

PROGRAM CONTINUATION

AFTER COMPLETING AN ASSESSMENT SESSION AND AN INITIAL TREATMENT PLAN, IF NECESSARY THE REGISTERED DIETITIAN WILL DISCUSS PROGRAM CONTINUATION WITH THE PATIENT

REBOOK ANOTHER TREATMENT PLAN BUNDLE (DISCUSS WITH THE RD)

or
1 - HOUR FOLLOW-UP SESSION

  • 1 Licensed Registered Dietitian session (up to 1-Hour in length) conducted via Webcam, Telephone, or Email Counselling
  • This follow-up session is only available for patients who have already had a treatment plan constructed by a Registered Dietitian in a previous session.

  • Supporting resource documents included for patients (sent via-email)

$110.00 + HST (paid in full at end of session - before exiting)

Please note:

**All additional work outside of the services above are billed at a rate of $110.00 + HST, per billable hour. The RD will get a confirmation from the patient before proceeding with any additional work.  (e.g. renal diet assistance, tube feeding, consulting with other health practitioners, additional communications with the patient that falls outside of the services paid, etc.)

** Registered Dietitians at the clinic can counsel only one patient at a time in a session.

** Please note, Registered Dietitians cannot diagnose medical conditions (this is the purview of a Physician in Ontario). Dietitians or Clinic Managers may request a Physician referral depending on the patient situation, at their discretion.
